INTRODUCTION

Cardiovascular magnetic resonance (CMR) is the current cornerstone imaging modality to evaluate cardiac masses and to differentiate thrombus from other findings1. Caseous calcification of the mitral valve annulus (CCMA) is a rare variant of annular calcification occurring in 0.06% of patients of all ages who undergo echocardiography2. This disorder appears in echocardiography as a large spherical mass-like calcification with a central echo-lucent area that may be leading to diagnostic errors. In fact, it should be differentiated from an abscess, tumor and thrombus. The present case refers to a patient with a mass-like calcification of the mitral annulus disclosed occasionally during a heart failure work up.
