ABSTRACT Hypertension (HTN) and HTN-related complications remain a major disease burden in Asian countries and among patients of Asian descent living worldwide. Asians generally have a higher salt sensitivity compared to Caucasians, complicated even more by the fondness for salty foods. The increased salt intake and salt sensitivity likely contribute to the Asian phenotype of HTN, which is characterized by a greater blood pressure (BP) variability, more isolated nocturnal HTN, higher early morning BP surge and a higher prevalence of masked HTN, which is often missed by the clinician. These factors can lead to stroke which is noted in the early morning hours and other cardiovascular events. Diuretic-based therapy along with lifestyle intervention should be considered a mainstay while managing hypertension in Asian patients and those of Asian descent.
